At Call To Compassion, we embrace the opportunity to collaborate with families who exhibit a heart of generosity and are eager to open their doors and extend the warmth of their homes to adults with intellectual disabilities. We serve families who already support these individuals in their homes as family members through our Family-Shared Living program. Additionally, we offer Non-Family Shared Living for those who are not related but wish to provide a caring environment. This is a prime example of leading by example and demonstrating the power of love in action. Here's how the partnership works:
Recruitment and Training: Call To Compassion works with the Sierra Regional Center to recruit and train potential shared living providers. They ensure that providers meet specific qualifications and standards required to offer care and support in a home setting.
Assessment and Matching: The Sierra Regional Center conducts assessments of individuals to determine their needs and preferences. Call To Compassion then uses this information to match individuals with compatible shared living providers who can meet their specific needs and preferences.
Ongoing Support and Monitoring: Call To Compassion provides continuous support and monitoring to both the shared living providers and the individuals in their care. This includes regular check-ins, training updates, and ensuring compliance with state regulations and program standards.
Resource Coordination: Call To Compassion coordinates with the Sierra Regional Center to provide access to additional resources and services, such as medical care, educational opportunities, and community activities. This ensures that individuals receive a holistic approach to their care.
Advocacy and Mediation: Call To Compassion acts as an advocate for both the shared living providers and the individuals, addressing any concerns or issues that may arise. They mediate to resolve conflicts and ensure that the living arrangement remains beneficial for all parties involved.
Documentation and Reporting: Call To Compassion maintains detailed records and documentation of the care provided, progress made, and any incidents or changes in the individual's condition. They report this information to the Sierra Regional Center to ensure transparency and accountability.
Quality Assurance: Both Call To Compassion and the Sierra Regional Center engage in regular quality assurance activities, such as evaluations and feedback sessions, to continuously improve the program and ensure that it meets the needs of the individuals served.
By working together, Call To Compassion and the Sierra Regional Center create a robust support system that enhances the quality of life for individuals with developmental disabilities, fostering independence and community integration.
